Short Bowel Syndrome QA with Samuel Alaish MD

MUSIC Short bowel syndrome isa condition of which patients either have lost a large amountof their intestine, or they were born with an insufficientamount of intestine, or their intestine just simplydoesn't work well enough. And the end result is they can'tabsorb enough nutrients from eating, or from feedingthrough a Gtube or a Jtube. To the point that they need,

supplemental nutritionthrough an IV line. MUSIC There are a variety of causes. The causes are differentin children and adults. In kids andbabies in particular, the number one cause is necrotizingenterocolitis, which is a severe infection that's most commonin premature infants. And this infectioncan be devastating.

The intestine can get diseased, and then frankly get necrotic anddie. And then the surgeonhas no choice but to take out the diseased segmentto save the baby's life. But then the children are leftwith short bowel syndrome. Other kids are bornwith an insufficient amount of intestine. That's known asintestinal atresia.

Still, other kids may havea normal amount of intestine at birth, and it works well fora month or two, or even a few years, andthen one day it just twists. And that's because itwasn't configured properly when they were insidetheir mom in the womb. That condition isintestinal malrotation with midgut volvulus. And the problem with the twistis that they could lose most,

if not all,of their small intestine, and be left with shortgut syndrome. Other kids will have enoughlength of intestine but it won't work. One disease isHirschsprung's disease, where the intestine'snot innervated properly. That can infect a small portionof the intestine normally, but it can also infecta large portion,

if not all of the intestine. And those kids will needthat intestine resected, and then they're left withshort bowel syndrome. Another cause that we seein babies is gastroschisis, which is a hole ininterior abdominal wall, next to the umbilical cord. And when the babies are inuterotheir intestine will poke out through that hole.
